1. General Information
​
Longevity GP is a privately operated general practice clinic led by Dr Colin Coward, a registered General Practitioner (GP) in Australia. We provide medical services including general health consultations, preventive care, lifestyle optimisation, and longevity-focused health assessments and follow up.

2. Introduction
These Terms and Conditions (“Terms”) apply to the Longevity GP 12-Month Membership Program both Regular and Premium (“Membership”) offered by Longevity GP, operated by Dr Colin Coward (ABN 44690005700 ) in South Fremantle, WA.
By enrolling, you agree to these Terms.

3. Membership Inclusions
The Allied Membership provides:
​
• One initial 90-minute clinic consultation with Dr Colin Coward
• 5 video consultations with a nurse coach (20–30 minutes each).
​
The Regular Membership provides:
• One initial 90-minute clinic consultation with Dr Colin Coward
• 11 video consultations (20–30 minutes each) with Dr Colin Coward
The Premium Membership provides:
• One initial 90-minute clinic consultation with Dr Colin Coward
• 11 video consultations (20–30 minutes each) with Dr Colin Coward
• Direct app messaging to Dr Colin Coward with a typical response time within 24 hours.
Additional services (e.g., VOâ‚‚ Max Testing, Base Fitness Checks, Home Consultations, extra Video Consultations or Clinic Consultations) are available separately and are not included in the Membership fee.
Investigation, treatment and allied health practitioner costs are not included.

4. Fees & Payment
​
• The Membership fee is as advertised on longevitygp.com.au for 12 months, payable upfront at commencement.
• The fee covers the package of services listed above.
• Fees are non-refundable, except where required under Australian Consumer Law.
• Membership is non-transferable.

5. Medicare Rebates
​
• Some consultations may be eligible for Medicare rebates under appropriate item numbers. These items will be made known to you and are kept by Dr Colin Coward.
• Any Medicare billing by Dr Colin Coward is separate from the Membership fee.

6. Appointments
​
• Consultations must be booked in advance via the Longevity GP online booking system.
• Missed or cancelled appointments with less than 24 hours’ notice are deducted from the membership quota for that service.
• Rescheduling is subject to availability.

7. Direct Messaging
​
• Messaging is available for non-urgent matters only.
• Clinical issues requiring urgent attention should be directed to 000 or your nearest emergency department or urgent care unit.
• Response time is typically within 24–48 hours during business days, though this may vary.

8. Duration & Renewal
​
• Membership is valid for 12 months from the start date.
• At the end of the term, patients may re-enrol in a new Membership at the then-current fee.
• Longevity GP reserves the right to modify Membership inclusions or pricing for future periods.

9. Limitations
​
• Membership does not replace your relationship with your regular GP for general or urgent medical care.
• Longevity GP services are focused on longevity, preventive and lifestyle medicine.
• Medicare and AHPRA guidelines restrict the scope of what can be delivered under this program.

10. Privacy & Data
​
• Patient information is managed in accordance with the Privacy Act 1988 (Cth) and Longevity GP’s Privacy Policy.
• Regarding mobile phone app instant messaging, the service is not fully compliant with the requirements of Australian medical information storage. The member will be informed of this verbally and in writing, and must consent to this vulnerability prior to any instant messaging service.
Patients must consent to using Threema Work communication method for the instant messaging service.
• Instant messaging is conducted using Threema Work.
• Threema is anonymous and encrypted, hosted in Switzerland. https://threema.com/en/faq/special-because
• Messages are summarised and stored in MediRecords.
Longevity GP will download instant messaging threads on a monthly based and upload them to MediRecords which is a medical data compliant, secure, electronic medical record.

11. Termination
​
• Longevity GP may terminate Membership if:
• There is non-payment of fees.
• A patient repeatedly misses appointments.
• Clinical boundaries or safe practice standards are breached.
• Patients may terminate their Membership early, but fees will not be refunded except as required by law.

12. Liability
​
• To the extent permitted by law, Longevity GP excludes all liability for any loss or damage arising from use of the Membership services, except where such liability cannot be excluded under Australian law.
​
13. Governing Law
These Terms are governed by the laws of Western Australia. Any disputes will be subject to the exclusive jurisdiction of the courts of Western Australia.

14. Code of Conduct
​

We maintain a respectful, inclusive environment and expect the same from all patients.

Aggressive, threatening, or abusive behaviour will not be tolerated and may result in termination of the doctor-patient relationship.​
