Terms and Conditions – Longevity GP

1. General Information
​
Longevity GP is a privately operated general practice clinic led by Dr Colin Coward, a registered General Practitioner (GP) in Australia. We provide medical services including general health consultations, preventive care, lifestyle optimisation, and longevity-focused health assessments.

2. Appointments

Appointments can be made via phone, our website, or in person.

If you are unable to attend, we kindly ask for at least 24 hours' notice. Cancellations with less notice may incur a fee.

Missed appointments without notice may result in a fee and/or refusal of future bookings.

3. Fees and Billing
​

Longevity GP is a mixed-billing clinic. Some services may attract Medicare rebates; others may be privately billed.

Payment is required at the time of consultation unless otherwise arranged.

A list of common fees is available upon request and on our website.

Fees for extended consultations, reports, procedures, and non-Medicare services (e.g., preventative health reviews, functional testing) will be advised beforehand.

4. Medicare and Health Insurance

Eligible services may be claimed through Medicare where applicable.

It is the patient’s responsibility to ensure their Medicare details are up to date.

We do not directly bill private health insurers, but receipts can be provided for self-claiming.

5. Telehealth

Telehealth consultations are available to eligible patients.

Patients must have had a face-to-face consultation within the past 12 months to qualify for Medicare rebates unless exempt under current Medicare guidelines.

You agree to ensure a private, quiet environment for telehealth consultations and understand that technical issues may arise.

6. Results and Follow-Up

Patients are responsible for following up on their test results.

We will attempt to contact you for urgent or significant abnormal findings. However, we encourage you to book a review appointment to discuss any test results.

7. Prescriptions and Referrals

Repeat prescriptions and referrals require a consultation to ensure ongoing clinical appropriateness.

Requests made without a consultation may not be fulfilled or may attract an administrative fee.

8. Confidentiality and Privacy

We comply with the Australian Privacy Principles under the Privacy Act 1988.

Your health information is kept confidential and is only shared with other providers involved in your care, unless you give written consent or we are legally required to do so.

A copy of our Privacy Policy is available upon request and on our website.

9. Clinical Scope

Longevity GP does not provide emergency services. In an emergency, dial 000 or present to your nearest hospital.

We do not prescribe Schedule 8 medications (e.g., opioids, benzodiazepines) for chronic use unless under specialist supervision.

10. Health Optimisation and Longevity Services
​

Certain longevity-focused assessments, tests, and treatments may not be covered by Medicare and may be considered complementary or investigational.

These services will be explained in full and require your informed consent before proceeding.

11. Code of Conduct
​

We maintain a respectful, inclusive environment and expect the same from all patients.

Aggressive, threatening, or abusive behaviour will not be tolerated and may result in termination of the doctor-patient relationship.​
