Terms & Conditions

1. General Information

Longevity GP is a privately operated general practice clinic led by Dr Colin Coward, a registered General Practitioner (GP) in Australia. We provide medical services including general health consultations, preventive care, lifestyle optimisation, and longevity-focused health assessments and follow up.

2. Introduction

 

These Terms and Conditions (“Terms”) apply to the Longevity GP testing plans offered by Longevity GP, operated by Dr Colin Coward (ABN 44690005700 ) in South Fremantle, WA.

By enrolling, you agree to these Terms.

 

3. Testing Package Inclusions

The testing pack price covers the cost of investigations stated within the testing pack as well as a follow up analysis and feedback session with the GP and with the Exercise Physiologist (EP).  It does not cover the cost of clinical or exercise physiologist support to deliver the recommendations made at the feedback advice sessions.  This can be done through the purchase of "Support Packages".

 

Medicare + DEXA:

  • DEXA Body Composition Scan provided by Perth Radiology Clinic, Hollywood

  • Eligible Medicare Bloods provided by Clinipath Pathology

  • Follow up 30 minute GP consultation

Foundation Testing Package

 

GP will refer for: 

  • Expanded clinical bloods 

  • Omega-3 index

  • DEXA body composition

  • 12-lead ECG

  • Cognitive baseline (Cognigram)

O2 Active Exercise Physiology (EP) will provide:​

  • VO2 + lactate threshold.   (VO2 max testing is ideally done using the VO2 master equipment with exercise escalating protocol up to zone 5 and heart rate maximum.  If this is deemed not safe for the individual other exercise testing will be used to predict the VO2 max)

  • Force plate & grip strength

  • Movement & mobility screen

  • Spirometry

  • Resting metabolic rate (ECAL)

"Six-month retest" means tests, where meaningful change can be expected, can be retested after mutual discussion between client and GP or exercise physiologist.  Tests will not be repeated if they are not useful or carry a risk of harm.

2 GP & 2 EP results sessions (1 following initial testing the 2nd after the retest).

 

 

The Performance Package:

 

Includes all of Foundation Testing Package plus:​​

  • CT coronary angiogram with calcium score where clinically indicated

  • Targeted risk-based MRI

  • Carotid ultrasound

  • Echocardiogram

  • 24-hour Holter / HRV

  • Low-dose CT and targeted imaging

  • 14-day continuous glucose monitoring

  • Home sleep study

  • Microba microbiome

  • TruAge epigenetic age

4. Fees & Payment
  1. The testing pack fee is as advertised on signal-health.com.au, payable upfront at commencement.

  2. The fee covers the package of services listed above as well as the stated follow up consultation(s).

  3. Fees are non-refundable, except where required under Australian Consumer Law.

  4. Purchase is non-transferable.

 

5. Medicare Rebates
  1. Some GP consultations may be eligible for Medicare rebates under appropriate item numbers.   Medicare stipulates that GP video consultations can only attract rebates if there has been a face to face consultation with that GP within the preceding 12 months and if the relevant item number criteria have been satisfied.

6. Appointments
  1. Consultations must be booked in advance via the Longevity GP online booking system.

  2. Missed or cancelled appointments with less than 24 hours’ notice are non refundable or, if applicable, will be deducted from the testing package quota for that service.

  3. Rescheduling is subject to availability.

7. Concierge Package 
  1. Direct messaging, phone and video calls are available for non-urgent matters only. 

  2. Instant messaging carries the Privacy limitations stated in 10.2

  3. Clinical issues requiring urgent attention should be directed to 000 or your nearest emergency department or urgent care unit.

  4. Response time will be as soon as reasonably possible with an absolute requirement of contact within 24 hours.

  5. The expectation is for continuity of care, meaning the same GP provides all GP services 24/7.  This has the exception of 6 weeks annual leave, or unforeseen sickness, and appropriate cover to the same level of service will be arranged.  The client will be fully informed throughout.

 

8. Duration & Renewal
  1. Testing Packages are valid for 6 months from the start date.

  2. At the end of the 6 months, if the GP feels longer is needed, then this end point can be flexible.

  3. Longevity GP reserves the right to modify testing package inclusions or pricing for future periods.

 

9. Limitations
  1. Testing Packages do not replace your relationship with your regular GP for general or urgent medical care.

  2. Longevity GP services are focused on longevity, preventive and lifestyle medicine.

  3. Medicare and AHPRA guidelines restrict the scope of what can be delivered under this program.

  4. Some investigations (e.g. whole genome sequencing, epigenetic clocks, biological age testing, microbiome analysis, metabolomics, proteomics) are evolving technologies intended to supplement, not replace, conventional medical assessment, and that clinical utility may change as evidence develops.

  5. No guarantee of health outcomes.  Recommendations by any body under Longevity Practice Pty. Ltd.  are evidence-informed but no particular improvement in lifespan, healthspan, disease prevention, or biomarker changes can be guaranteed.

10. Privacy & Data
  1. Patient information is managed in accordance with the Privacy Act 1988 (Cth) and Longevity GP’s Privacy Policy.

  2. Regarding mobile phone app instant messaging, and email, the service is not fully compliant with the requirements of Australian medical information storage.  The member will be informed of this verbally and in writing, and must consent to this vulnerability prior to any instant messaging or email service. 

  3. Longevity GP will record any instant messaging threads and emails on a monthly basis and upload them to MediRecords which is a medical data compliant, secure, electronic medical record.   After this the message threads on other platforms will be deleted.

  4. We use Heidi AI, an Australian healthcare-focused clinical documentation tool, during consultations to assist with creating and updating your clinical notes. All information processed by Heidi AI is handled in accordance with applicable Australian privacy legislation and healthcare data security standards.  Your clinician reviews and approves all documentation and you may withdraw your consent to this at any time.

11. Termination
  1. Longevity GP may terminate the package plan if:

  2. There is non-payment of fees.

  3. A patient repeatedly misses appointments.

  4. Clinical boundaries or safe practice standards are breached.

  5. Patients may terminate their package early, but fees will not be refunded except as required by law.

12. Third-Party Testing and Service Providers

As part of your care, we may recommend or facilitate investigations, laboratory testing, imaging, genetic testing, physiological assessments, wearable technologies, or other services provided by independent third-party organisations ("Third-Party Providers").

These Third-Party Providers operate independently of Longevity GP. Unless expressly stated otherwise, they are not our employees, agents or representatives, and they are solely responsible for the services they provide, including the collection, analysis, reporting and storage of your information.

Our role is limited to advising on the clinical appropriateness of testing, assisting with referrals where appropriate, and interpreting results within the context of your healthcare.

By proceeding with testing, you acknowledge and agree that:

  • you may be required to enter into separate agreements with Third-Party Providers and accept their terms, conditions and privacy policies;

  • fees charged by Third-Party Providers are determined by those providers and may change without notice;

  • turnaround times, availability of tests, methodologies, quality assurance processes and reporting formats are determined by the Third-Party Provider;

  • Longevity GP does not warrant or guarantee the accuracy, completeness, availability or timeliness of any testing or reports produced by a Third-Party Provider;

  • where samples are collected, transported or analysed by Third-Party Providers, those providers are responsible for the handling and security of those samples and associated data.

To the maximum extent permitted by law, Longevity GP is not liable for any loss, damage, delay, error or omission arising from the acts or omissions of any independent Third-Party Provider. Nothing in these Terms excludes, restricts or modifies any rights or remedies that cannot lawfully be excluded under the Australian Consumer Law or any other applicable legislation.

If concerns arise regarding the performance of a Third-Party Provider, we will make reasonable efforts to assist you in communicating with that provider and, where clinically appropriate, recommend alternative testing options.

 

13. Governing Law

 

These Terms are governed by the laws of Western Australia. Any disputes will be subject to the exclusive jurisdiction of the courts of Western Australia.

14. Code of Conduct

  1. We maintain a respectful, inclusive environment and expect the same from all patients.

  2. Aggressive, threatening, or abusive behaviour will not be tolerated and may result in termination of the doctor-patient relationship.
